SCHOLARSHIP CRITERIA JODY ASMUSSEN MEMORIAL SCHOLARSHIP
Antelope Memorial Hospital Board of Trustees is offering Scholarships, The Antelope Memorial Hospital Jody Asmussen Memorial Scholarship, to assist young people interested in health-related fields of education. Awards are granted without regard to race, color, creed, religion, gender, disability or national origin. Recipients have no obligation to Antelope Memorial Hospital. Antelope Memorial Hospital is offering a five hundred ($500.00) dollar scholarship for a graduating senior from each of the area senior classes who plans to pursue a health-related career. The Scholarship will be sent to the school being attended after the recipient completes one semester of school and is enrolled in the second semester toward a health-related career. The area schools are:
Clearwater Public.. Neligh-Oakdale Public Elgin Public Ewing Public
Orchard Public Pope John Central Catholic (Elgin) Elkhorn Valley (Tilden)
Eligibility
→ Applicants must be a 2022 graduating senior in one of the above-mentioned high schools. > Applicants must have attended the current high school for at least two (2) years. > Applicants must plan to enroll in a full-time healthcare-related undergraduate course of study at an accredited 2
to 4-year college, university, or vocational-technical school.
Submitting an Application
► Interested students must complete the application and mail to Diane Carlin, CEO, Antelope Memorial Hospital,
102 West 9th Street, PO Box 229, Neligh, NE 68756 before April 18, 2022. > Applicants should give out two (2) evaluations to be returned to Diane Carlin, CEO, Antelope Memorial Hospital,

Applicant should submit a recent "close-up" photograph (put your name on the back). > Applicant should submit written verification of acceptance from the school the applicant will be attending.
Selection of Recipients
> Academic record > Participation in school and community > Statement of goals and aspirations > References
Awards
>> Up to seven (7) awards, of $500 each, from each of the area high schools mentioned above
► Recipients are eligible to receive only one (1) award
Awards are not renewable → Awards are for high school seniors to be used in a health-related undergraduate course of study
Payment of Scholarships
>> Checks will be made payable to the school and deposited to the student's account after one semester is
successfully completed and the student is enrolled in the second semester.
► Written verification of acceptance from the school the recipient will be attending, > Complete address of the school the recipient will be attending.
