D. J.'s Hero Award - Salvation Army

D.J.'s Hero Awards- please read the following criteria - if you think you meet the criteria and would like to be nominated please see Mrs. Bode (we will discuss criteria)

The D.J.'s Hero Award scholarship is unique in that it honors the an œunsung hero who has overcome challenges and demonstrated leadership and commitment to his/her community.

Nominee:  The nominee (high school senior) must provide an essay (not to exceed 250 words) on why they are deserving of this very special recognition and scholarship.

Nominator:  Must be a counselor, teacher, friend, mentor, pastor etc.  The nominator cannot be a relative of the student.  The student cannot nominate themselves.
On a separate sheet of paper, the nominator must complete the following questions, using detailed and very
specific examples (list questions numerically). Failure to answer all questions in complete detail
will disqualify the nominee. Applicant must be a citizen of the United States. Scholarship funds must
be used within the United States.
1. Describe in detail how the nominee demonstrates courage by acting independently.
2. Explain in specific detail how the nominee overcomes challenges/adversity.
3. Please identify the following: (a) the personal goals of the nominee, (b) nominee's greatest achievement in life and (c) how he/she expresses enthusiasm for life.
4. Demonstrate the nominees commitment to others and the community (please avoid resume-like
listings of volunteer projects).
The nomination detail should not exceed two typed pages (must use at least an 11 point font size).
